The Maimonides Medical Center Department of Population Health provides support and management services to the Community Care of Brooklyn Performing Provider System, Community Care of Brooklyn Independent Practice Association, Brooklyn Health Home, Vital Brooklyn, and various other population health programs operating in Brooklyn, NY.

Working closely with CCB’s web based technology product vendors and users as the subject matter expert, the IT Implementation Specialist will be responsible for the implementation, use, support, and enhancement of CCB care coordination and population health systems. The position will require specialists to travel to CCB healthcare and community based social service partner sites throughout Brooklyn to prepare, deploy and support CCB web-based information technology systems. The IT Implementation Specialist will provide training and support for all users of CCB web based technology solutions, either individually, with technology vendor support, and/or as part of cross-functional teams, both on-site and remotely via webinars and conference calls.
Bachelor’s degree or equivalent work experience. Minimum 1 year of experience in implementation and/or support of healthcare information technologies.
