Community Care of Brooklyn is staffed by the Central Services Organization (CSO) of Maimonides Medical Center, the lead entity and fiduciary for CCB. Maimonides formed the CSO to manage the DSRIP project, the Brooklyn Health Home (BHH) and other population health initiatives.
Current open positions within the CSO are described below. IT Implementation Specialist
Working closely with CCB’s web based technology product vendors and users as the subject matter expert, the IT Implementation Specialist will be responsible for the implementation, use, support, and enhancement of CCB care coordination and population health systems. The position will require specialists to travel to CCB healthcare and community based social service partner sites throughout Brooklyn to prepare, deploy and support CCB web-based information technology systems. The IT Implementation Specialist will provide training and support for all users of CCB web based technology solutions, either individually, with technology vendor support, and/or as part of cross-functional teams, both on-site and remotely via webinars and conference calls.
