Comprehensive Pediatrics (full interview) - Comprehensive Pediatrics has three NCQA PCMH Level 3-recognized practice sites and has been a close and trusted partner to CCB over the course of the DSRIP program. CCB interviewed Danny Eugene, Business Operations Manager at Comprehensive Pediatrics to learn more about their approach to providing effective patient education in a hub of cultural diversity, helping patients with chronic conditions, transitioning to value-based payment, and how teams can work together to drive change.
LegalHealth, a division of the New York Legal Assistance Group (full interview) - Following a study on the impact of social services on healthcare utilization and costs, CCB partnered with LegalHealth to expand an existing legal clinic model to provide low-income patients in Brooklyn with legal assistance. Melina Somoza, Esq. has since been helping patients referred by CCB network providers navigate an intimidating legal system, empowering them to address social or economic factors affecting their daily lives, and enabling them to focus on their health. Ms. Somoza introduces herself in this feature and provides clarity on the services that she provides at the CCB LegalHealth Clinic.
Bridge Back to Life Center (full interview) - CCB has partnered with Bridge Back to Life to pair Certified Recovery Peer Advocates (CRPAs or “peers”) with our partners. CCB has provided Bridge financial support to employ peers and place peers at specific sites in Brooklyn with the goal of connecting more patients with substance use disorder (SUD) to treatment and continuing to engage them throughout rehabilitation to reduce their need of hospital services and encourage full recovery.
